Adding Custom Forms to Calendars
A custom form on a calendar collects extra details before the appointment is booked. The visitor picks a time first, then fills out the form. Answers that are mapped to contact fields are saved on the contact when they click Book Appointment. If they leave before that button, nothing is written to the contact.
Show and hide rules on the form still work. Disqualify rules run before the appointment is created. A disqualified visitor does not get an appointment, and no payment is taken. The form can send them to a redirect address or show a custom message if you configured one.
The confirmation setting chooses what they see after submit:
- Default uses the calendar's usual confirmation.
- Redirect URL sends them to an address you set.
- Use custom form rules follows the form's redirect or custom message. If the form has neither, the booking falls back to the default confirmation.
Attach a form
Build and save the form first under Sites > Forms. It needs at least one field.
- Open Calendars, then Calendar Settings.
- Find the calendar and click Edit.
- Open the Forms & Payment tab.
- Under Select Form, choose the form.
- Click Save.
Only one form can be attached to a calendar. The same form can be used on other calendars, but you attach it on each calendar yourself. It is not copied across calendars automatically.
If the form is missing from the list, confirm it is saved under Sites > Forms, that it has a field, and refresh the calendar settings page.
Edits to the form show up on every calendar that uses it. Do not delete a field that a workflow or a contact record depends on.
What the answers are for
Mapped fields land on the contact, so workflows can use them. Terms or consent checkboxes can sit on the same form. Unmapped questions are still in the form submission, but they will not fill a contact field until you map them.
